· Copy of life and health insurance license

· Appointment fee $40 – make check payable to Gerber Life Insurance

· Producer Agreement

Corporate Appointment

· Producer’s Information Questionnaire

· Copy of life and health insurance licenses (corporate principal)

· Copy of Corporate license

· Appointment fee $40 – make check payable to Gerber Life Insurance

· Producer Agreement (corporate information)

· Assignment of Commissions form
